POSITION SUMMARY:
Dementia Care Coordinator will provide coordination of care to individuals with dementia, their families and caregivers. Performs professional human service work that contributes to the efficient and effective operation of dementia care services across the Office on Aging and Independence. The care coordination will include, but not be limited to, options counseling, education on dementia, dementia screenings, information and referrals to community-based organizations, care transitions coaching, organization of support groups, behavioral symptom management training and expert consultation and eligibility assistance.

***This is a part-time, benefited position working an average of not more than 30 hours per week. This position will serve at the pleasure of the Appointing Authority.***

$26.83 - $29.38

CLASS DESCRIPTION:
Performs professional level human services work, under general supervision from an administrative or technical superior. Work includes human service, administrative, and counseling work; providing counseling and programmatic services to the special populations; and providing assistance in identifying the population at risk, its needs and how to meet those needs. Organizes and publicizes large events for citizens or special groups. Work also includes providing services needed to carry out human service programs or the operation of a medium sized center with extensive regional responsibility and concurrent programming.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Include is the following. Other duties may be assigned.
  • Provides support to caregivers and families on issues related to caregiving and Alzheimer's disease and related dementia (ADRD).
  • Promoting awareness of ADRD, enhancing memory screening, and facilitating caregivers' access to essential community resources.
  • Provide dementia-specific consultation, options counseling, information and assistance, referrals, and screenings utilizing the AD8 assessment tool.
  • Conduct dementia care consultations for individuals living with dementia and/or their caregivers. Provide referrals to programs that engage individuals who demonstrate symptoms of dementia that interfere with activities of daily living, regular exercise, and social activities
  • Conduct outreach and education on Alzheimer's Disease and related dementias (ADRD), navigation of long-term care, and effective communication with healthcare providers. Raise awareness of Office on Aging and Independence programs and services to include Caregiver Support Program, Community Living Program, Maryland Access Point (MAP) and dementia support services.
  • Coordinate dementia care services across the Office on Aging and Independence to include evidence-based or evidence-informed programs targeting caregiver of individuals living with dementia.
  • Coordinate or facilitate support groups such as Memory Cafe'.
  • Conduct dementia care consultations for individuals living with dementia and/or their caregivers. Provide referrals to programs that engage individuals who demonstrate symptoms of dementia that interfere with activities of daily living, regular exercise, and social activities.
  • Coordinate Dementia Live trainings to the community and other community partners supporting caregivers and care recipients.
  • Collect relevant data to complete federal and state reports.
  • Participate in Maryland Department on Aging statewide and regional meetings and training programs.
  • Participate in activities related to Emergency Management as requested, including work outside normal business hours.
  • Maintain knowledge of the latest research on dementia, evidence-based interventions, and new innovations for individuals living with dementia and their caregivers.

MINIMUM REQUIRED EDUCATION/EXPERIENCE

Bachelor's degree or equivalent combination of education and experience.

PREFERRED EDUCATION, KNOWLEDGE, SKILLS AND ABILITIES:

  • Budget experience and experience with managing state and/federal grants.
  • Experience developing collaborative community partnerships.
  • Program development and evaluation experience.
  • Database management skills.
  • Understands the role of Howard County Governmental and other private or public programs in aiding the older population and persons with disabilities.
  • Bachelor's degree in social work, Gerontology, Human Services or related field with two (2) or more years' experience a human services field, working with individuals with dementia and their families and caregivers.
  • Experience in case management or social work.
  • Strong interpersonal skills, excellent oral and written communication skills and the ability to work well with an ethnically diverse population.
  • Strong computer proficiency especially with Microsoft Office (Word, Excel, PowerPoint), database management and spreadsheet applications necessary.
  • Proficiency in virtual platforms and use of technology for human services delivery.
