HealthRIGHT360’s Advanced Practice Provider (APP) Residency is a two-year training program in San Francisco, California for recently graduated Physician Assistants and Nurse Practitioners who are passionate about providing primary care services to medically underserved communities. Commonly called an “APP Fellowship” at other institutions, the purpose of the program is to adequately equip new APPs to provide high-quality, trauma-informed, and evidence-based primary care in a compassionate, culturally competent way to individuals who are racially, ethnically, and socioeconomically diverse. Through the residency, we hope to improve access to quality medical care for the underserved, while simultaneously building a sustainable workforce that is appropriately trained to care for this complex patient population.  With a new academic partnership with the University of California Davis, HealthRIGHT360’s program offers residents a diversity of learning opportunities including didactics, skills learning, mentorship, preceptorship, and specialty rotations. During the first month of the program, residents will participate in an intensive boot camp to shore up clinical basics and receive site-specific orientations such as EMR training, practice management, team-based care, quality improvement, and shadowing of different departments. Following the HealthRIGHT360 boot camp, residents will participate in a week-long didactic education experience at UC Davis in Sacramento again focusing on skill refinement and didactic presentations.   After orientation, residents will start seeing patients in the primary care, underserved clinic (FQHC) setting in San Francisco and San Mateo. Residents will start with a small patient panel and slowly build their practice throughout the course of the two-year training program. They will receive formal precepting from MDs, DOs, NPs, and PAs in Primary Care, as well as specialty care services. Each resident will receive 1:1 weekly mentoring and quarterly evaluations. Residents will participate in weekly didactics as a cohort through UC Davis and HealthRIGHT360. Every quarter, residents will also travel to UC Davis for 3-day, in-person skills training. Example trainings include Nexplanon insertion and removal, joint injections, incision and drainage, and EKG mastery.  Nearing the end of the first year, residents will have the opportunity to participate in specialty rotations. Specialty rotations will include those that support care for our safety net population, including but not limited to addiction medicine, psychiatry, mobile health, Street Medicine, podiatry, HIV care, hepatitis C management, gender affirming care, wound care, and women’s health. Second year residents will continue building their own primary care practice, with the possibility to choose a focus track of specialty care. They will continue to receive mentorship and focused training. During the second year, residents will also participate in a year-long quality improvement project, attend leadership training, and will mentor first year students.  Cohorts of residents will consist of a minimum of two members. The Program begins in mid-September 2025. The Advanced Practice Provider Resident (APPR) is a salaried staff position that is engaged in the APP residency. Under the supervision of the APP Residency Director, the residents will have administrative support with scheduling and general operations of the program. Residents will have clinical supervision overseen by the APP Residency Director. Residents will work with “preceptors” who provide clinical support and teaching in direct patient care. Residents will be expected to participate in didactics, trainings, and preceptorships. KEY RESPONSIBILITIES  Clinical Responsibilities   Residents will be trained on and expected to:  Provide comprehensive care, within the scope of credentialed privileges under the clinical teaching of the preceptor and/or clinical supervision of the APP Residency Director and/or Medical Director. Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses. Medical examination will include the interpretation of all routine laboratory tests. Ensure appropriate follow-up for all aspects of ordered care (e.g., specialty referrals, laboratory results, radiology reports, etc.). Diagnosis and treatment of illness and injury.  Consulting with the APP Residency Director, Medical Director, supervising physician, and/or preceptor for treatment referral options as needed. Maintain familiarity with the standardized procedures as agreed to by the preceptor/APP Residency Director/Medical Director and utilize them to guide care. Maintain current productivity standards as assigned by supervisor through appropriate scheduling and monitoring of no-show rate. Provide clinical leadership to support staff, including medical assistants, patient care coordinators, front office staff and receptionists. Work collaboratively with mental health staff to facilitate integration between primary care and mental health in order to improve patient outcomes. Along with colleagues, QI team, and preceptor, assist in continuous quality improvement through on-going problem/needs identification and problem solving. Obtain and maintain licensure, DEA registration and BCLS/CPR certification. Complete all mandatory trainings on an annual basis. Comply with all aspects of HealthRIGHT360 Policies and Procedures applicable to Medical Staff members as well as all agreements with insurers and other entities that contain provisions applicable to or affecting the duties and responsibilities of Medical Staff; and all applicable statutes, regulations, governmental agency guidelines, and non-governmental oversight body requirements that place requirements on HealthRIGHT360 or Medical Staff. Consistently arrive on time for all scheduled shifts and meetings. Maintain a full presence during assigned working hours, ensuring no early departures unless pre-approved by a supervisor. Perform other duties as assigned by supervisor to maintain a high level of patient-centered care. Administrative Responsibilities:    Maintains complete and detailed records of patients' health care plans and prognosis, documenting care within 72 business hours per policy and following charting protocols and applicable CA state licensing requirements. Upon completion of QI training, engages in one or more QI projects with the clinic team. Upon successful completion of year 1 of the residency program, serves as a mentor for the new year 1 residents.     Compliance responsibilities:    Understands and ensures compliance with policies and procedures to manage risk.   Ensures compliance with HIPAA, 42CFR regulations and all other funding mandates and licensing requirements.      Training responsibilities:    Attends and participates in requiredHealthRIGHT360 trainings and meetings.   Attends and participates in required UC Davis APP Fellowship trainings and meetings. Completes all didactic assignments, as assigned by HealthRIGHT360 and UC Davis. Engages in leadership training as assigned. Participate in regular evaluations, as assigned through HealthRIGHT360 and UC Davis.         EDUCATION AND KNOWLEDGE, SKILLSAND ABILITIES QUALIFICATIONS:  Education and Experience   Required:  Graduation from an accredited program for Nurse Practitioners or Physician Assistants at time of program start.  Applicants must have graduated no more than 18 months before the start of the program. Exceptions may be granted on a case-by-case basis. Clinical experience with and commitment to working with diverse client populations, including those experiencing homelessness and those with substance use disorders.  Desired: Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and culture backgrounds and economic status. Experience working successfully with issues of homelessness, substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.    Certification/Licensure  Required:  Possession of a current, valid California License to practice as a Family Nurse Practitioner/ Adult-Gerontology Nurse Practitioner OR Physician Assistant (can be in process of being obtained at time of application, must obtain before starting); AND Passing of national board examination (can be in process at time of application, must have completed before start); AND DEA certification (can be in process at time of application); AND NPI (can be in process at time of application, must obtain before starting); AND Current BLS certificate or recertification within 2 months of hire  Background Clearance   Required: Ability to obtain and maintain satisfactory background check.      Knowledge  Required:  Culturally competent and able to work with a diverse population Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications Experience with electronic medical record systems.  Desired:  Bilingual Spanish/English LGBTQIA Cultural competence  Comfort working on teams and with safety net populations.    Skills and Abilities  Required:  Professionalism, punctuality, flexibility, and reliability is imperative. Excellent verbal, written, and interpersonal skills. Integrity in handling sensitive information in a confidential manner. Action-oriented. Strong problem-solving skills. Excellent organizational skills and ability to multitask and juggle multiple priorities. Outstanding ability to follow through with tasks. Ability to work cooperatively and effectively as part of an interdisciplinary team and independently assume responsibility. Strong initiative, enthusiasm, and willingness to pitch in whenever needed. Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations.

Salary : $100,000 - $110,000
