Responsibilities
The role of the clinical pharmacist will be to provide excellence in patient care, patient safety, and adherence to regulatory requirements. There is also an opportunity to learn other specialties including pediatrics, oncology and critical care. Other responsibilities include :
- Daily patient profile review to ensure optimized medication therapy
- Rounding with an interdisciplinary team on selected units
- Attending codes, stroke alerts and traumas
- Pharmacokinetic dosing
- Warfarin management
- Patient education and transitions of care work
