Job Description:
The Certified Peer Specialist/Peer Care Manager is a key member of the CORE & HCBS team, working closely with the Director and Team to provide personalized support, services, and resource connections for Members in their home or community.
We employ a person-centered, trauma-informed care model that emphasizes empowerment, choice, and health and wellness goals to optimize quality of life and reduce symptoms of behavioral health conditions.
To succeed in this role, the Certified Peer/Peer Care Manager must have a New York Certified Peer Specialist (NYCPS) Provisional Certification within 3 months of hire. Additionally, they must have a valid driver's license, a satisfactory driving record, and own car. A satisfactory background check through fingerprinting search, including clearance from the NYS Justice Center, Protection of People with Special Needs Staff Exclusion List, NYS Medicaid Exclusion List, and NYS Office of Children and Family Services, is required.
