- Conduct face-to-face assessments and home visits to assess client needs and provide support.
- Coordinate and facilitate access to community resources and services.
- Maintain accurate and timely records of client interactions and progress.
Requirements
- Bachelor's degree in health and human services field.
- 2 years of related experience.
- Exceptional communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license and proof of insurance.
