About the Role:
The Field Care Manager, Behavioral Health plays a crucial part in assessing and evaluating members' needs to achieve optimal wellness. This role involves guiding members/families toward resources appropriate for their care and wellbeing.
Key Responsibilities
- Perform telephonic and face-to-face assessments to identify member needs and requirements.
- Ensure member progression towards desired outcomes by continuously monitoring care through assessments and/or evaluations.
- Create member care plans that align with department, segment, and organizational strategy.
- Collaborate with providers and community services to promote quality and cost-effective outcomes.
- Coordinate across the transdisciplinary care team to ensure seamless transitions of care.
Required Qualifications:
- Current Unrestricted Registered Nurse License in the state of Louisiana.
- At least 2 years of experience working as a behavioral health professional.
- Experience with behavioral change, health promotion, coaching, and/or wellness.
- Proficiency with Microsoft Office Programs, e.g., Word, Excel, & PowerPoint, shared systems, troubleshooting, and resolving general technical issues.
- Must reside and perform work in the state of Louisiana.
