Job Description:
The Field Care Manager, Behavioral Health is responsible for assessing and evaluating member needs to achieve optimal wellness. This role involves guiding members/families toward resources appropriate for their care and wellbeing.
Main Objectives
- Identify member needs and requirements to develop effective care plans.
- Collaborate with providers and community services to promote quality and cost-effective outcomes.
- Ensure seamless transitions of care across the transdisciplinary care team.
Qualifications:
- Current Unrestricted Registered Nurse License in the state of Louisiana.
- At least 2 years of experience working as a behavioral health professional.
- Experience with behavioral change, health promotion, coaching, and/or wellness.
- Proficiency with Microsoft Office Programs, e.g., Word, Excel, & PowerPoint, shared systems, troubleshooting, and resolving general technical issues.
