Provide physical therapy treatment and evaluations in a home health setting
Develop plans of care for patients
Participate in in-service programs and present in-service programs as assigned
Monitor patient status and compliance
Territory to cover primarily Glastonbury, Portland and East Hampton
- Must have excellent clinical and communication skills
- Experience providing Physical Therapy services
- Master’s degree in Physical Therapy
- State Licensure
