JOB SUMMARY: Under the supervision of the Director of Operations, the Community Supports Coordinator is responsible for providing detailed information regarding supports & resources available to aging populations offered by various public and private agencies, community groups, and non-profits.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Research, recommend, and refer clients to services, programs, and benefits available for adults age 55 and older.
- Act as point of contact for outside agencies and individuals seeking information on HSRC and other programs/resources available for adults age 55 and over.
- Facilitate referrals to programs and assists individuals with application processes.
- Develop and maintain collaborative working relationships with various public and private service agencies, community groups, non-profits, and potential referral sources.
- Participate in community workshops to develop relationships.
- Maintain data base and up-to-date information on various programs, criteria, and contacts for all resources.
- Assist individuals with questions and/or enrollment process for Medi-Cal programs.
- Interpret and ensure compliance with program requirements as well as applicable laws, regulations, policies, procedures, and contracts.
- Respond to referrals and requests for information in a timely manner via email, phone, and in person appointments.
- Communicate regularly and effectively with agency employees and participants in regard to the status of requests.
- Coordinate case management with other social services providers.
- Complete and maintain records, invoicing, and reimbursement requests for program and grant-related services.
- Perform home visits and off-site case management meetings.
- Conduct housing assessments and assist clients in locating and securing housing.
- Participate in the process of teaching, training, and mentoring new staff, student interns, and volunteers working within the program area.
- Acts in a manner consistent with agency values.
- Other duties as assigned.
Qualifications
EDUCATION &/or EXPERIENCE:
- High school diploma or GED equivalent required.
- Combination of higher education and/or at least one (1) year of experience coordinating programs or services required.
- Knowledge of the Housing and Urban Development (HUD) Public Housing Program and/or community-based housing programs, Medicare, or Medi-Cal programs preferred.
- Experience with or exposure to the unhoused community and social services care planning.
- Existing relationships with various Humboldt County community partners preferred.
- Bilingual preferred.
LICENSES OR CERTIFICATES:
- Valid California Driver’s License and personal auto liability insurance required. Personal vehicle in good driving/operating condition, insured for State minimum liability requirements that can be used for the applicable job functions noted above required.
PRE-SCREENING REQUIREMENTS:
- Requires clearance of a DOJ and FBI criminal history background check
- Requires Motor Vehicle Report verification.
- COVID-19 and Influenza (flu) vaccines preferred.

- Ability to represent the agency professionally and effectively in meetings with internal and external stakeholders.
- Ability to speak publicly and privately about agency programs and to participate in various outreach efforts as required.
- Strong basic computer skills (Microsoft Outlook, Word, Power Point, and Excel). Ability to implement, navigate, use, and maintain database software programs.
- Basic knowledge of computers and web-based applications such as timekeeping, training, and email programs.
SUPERVISORY REQUIREMENTS:
- This position has no Supervisory requirements.
PHYSICAL REQUIREMENTS:
- Physical abilities sufficient to move between different staff work areas, communicate with co-workers and the public, operate a computer, produce reports, talk on the telephone, and travel to other agency worksites.
