Position Title: Supportive Housing Service Coordinator
Report To: Housing Services Supervisor
Description: The Service Coordinator is responsible for providing support and case management to participants in HHSI’s Supportive Housing program. Their primary focus is assisting people to remain in housing in, connecting them to eligible mainstream resources and helping transition them from supportive housing to rent
Qualifications: At least one (1) year experience in social service experience either through work, volunteer or intern experiences. Experience working with vulnerable populations and those experiencing homelessness a plus.
Experience: Individual must possess demonstrated experience with crisis intervention, Trauma-Informed Care, and social work practice with chronically homeless individuals and/or individuals with disabilities (mental illness, substance abuse/addiction, physical disability, developmental disability, chronic health problems, etc.) Individual must also be capable of preserving the confidentiality of employee and participant records.
Preferred experience includes: working with people or families who have experienced homelessness and have disability of long duration including mental health, substance abuse, mental or physical impairment.

Responsibilities:
1. Commitment to Ethical and Effective Services
· Know and implement HHSI’s mission, policies, and procedures; and abide by all city, state, and federal laws, and regulations.
· Maintain trainings in suicide risk assessment, mandatory child and dependent adult abuse and neglect reporting, crisis intervention, and civil rights, housing discrimination, harm reduction, trauma informed care and housing first practices.
· Regularly evaluate a caseload of participant needs in designated program
· Adhere to HHSI’s agency values in all aspects of work: Compassionate Individuality, Dignified Empowerment, Innovative Inclusivity, Transparent Integrity, Sustainable Humanity, Humility in Service, and Unwavering Advocacy.
2. Case management:
· Meets with program participants and creates an Individualized Housing Plan with them. Frequency and location of meetings determined by program requirements and individual need.
· Completes a housing need, and mainstream resources assessment with participants.
· Completes any other mandated assessments per specific grant agreement or program policy and procedures
· Transports program participants to appointments, apartment showings and any other necessary locations based on their ISP
· Connects participants to mainstream benefits and assists them in applying when applicable
· Provides life skills training like laundry, navigating the bus system etc. for program participants
· Provides crisis intervention and advocacy for participants, as needed
· Follows evidence practice from Corporation for Supportive Housing including moving on strategies.
3. Documentation:
· Completes daily case notes, service transactions, and contacts then uploads them to Illinois Data Base System for Homeless management.
· Maintains current income verification, eligibility and services provided as required for specific grant agreement
· Maintains releases of information for all participants and external providers when applicable
· Completes fund requests for participant needs when applicable
· Refers participants to other programs at HHSI for which they are eligible, along with supportive documentation
4. Other Duties:
· Collaborates on case plans for high need participants with the Supervisor
· Case Conferencing weekly with Supervisor
· Maintains strong knowledge of community resources, especially as it relates to housing
· Regularly promotes the agencies’ mission and advocates for social justice
· Performs other duties, as needed
Schedule:
Full-Time. Must be able to work 40 hours per week.

The work environment for this position scattered-site housing with a variety of accessibility and environmental conditions; and an accessible shelter facility with moderate to occasionally loud noise levels. Environment includes donated food items, which may contain a variety of allergens. Position also regularly required to use janitorial supplies, such as disinfectants, laundry detergent, air fresheners, etc. Shelter, office, and scattered-site housing may occasionally house service and emotional support animals.

Pay: $18.10 - $24.37 per hour
