Position Summary
- This position provides patient care to the client in their home under the supervision of a Registered Nurse. Duties include but are not limited to : personal care and homemaking services essential to the patient's health care and comfort at home, including shopping, laundry, meal planning, prep, serving (Rx diet) and assisting with ADL's- bathing, dressing, grooming, toileting, mouth care, hair care, feeding, mobility, transfers, and ambulation. Supports the Mission, Philosophy and objectives of Hunterdon Hospice.

Required :
High School Diploma or Equivalent. Must have successfully completed a NJ BON approved 76 hour training & competency program and hold a current New Jersey Certificate from the Board of Nursing

1 year CHHA experience
Required :
Current NJ CHHA Certification from NJ Board of Nursing, Current Driver's license
CPR Certification
Required :
Completion of NJ CHHA Program with passing grade, must pass annual clinical competency based program and mandated 12 hour in-service education. Skills will be determined by direct supervision of a Registered Nurse / Supervisor as well as attendance and participation in 12 hour in-service education. (If attending / receiving education through another source will need to provide written evidence / documentation of attendance, hours of education, and content of education)
