Hunters Pond Rehabilitation and Healthcare is an amazing 128 bed skilled nursing facility with beautiful spacious rooms in both private and semi-private settings with private showers. We are located on the south side of San Antonio by Palo Alto College. Our commitment to excellence extends beyond delivering world class health care. Duties:
- Assists the Director of Nursing Services in managing and directing the Nursing Services Department.
- Provides direct nursing care as necessary.
- Makes rounds to ensure that nursing personnel perform their work assignments in accordance with acceptable nursing standards.
- Makes rounds with physicians as necessary.
- Communicates information to nursing personnel regarding new resident admissions and resident discharges and provides oversight of the admission and discharge process.
- Reviews nursing personnel medical record documentation to ensure that it is appropriately and accurately descriptive of the nursing care provided.
- Contributes to the development and maintenance of written nursing policies and procedures.
- Provides written and oral reports/recommendations to the Director of Nursing Services and Administrator, as necessary/required, concerning the operation of the nursing services department.
- Prepares for and participates in facility surveys (inspections) made by authorized government agencies.
- Develops and maintains nursing service objectives and standards of nursing practice in accordance with the nursing policies and procedures
- Assesses each resident’s needs and assists in the development of a treatment plan for nursing care to meet the daily treatment and care needs of the resident
- Participates in the interdisciplinary team and works cooperatively with team members to ensure the coordination of care and treatment for the residents are met
- Ensures that nursing personnel provide treatments, medications, diets, rehabilitative nursing care, activities of daily living, protective service, and training in self-care as stated in the care plan to ascertain that optimum comfort and care is provided
- Train nursing staff in the proper provision of direct care to residents
- Lead others through actions consistent with and serve our culture
Qualifications:
- Current RN or LVN license is required
- Minimum of 2 years of Long Term Care experience preferred
- Able to handle stressful situations in a calm and professional manner
- Capable of organizing tasks and managing multiple priorities
