- 40 hours per week; Monday through FridayDescription : Home visits with families on an as-needed basisWrite goals with families to help meet their needsSupports parents in accessing services and supportsEngages families in meetings, trainings, and activities within the programHelps parents to meet their children's health needs and requirements of the programRequirements : Bachelors Degree in Human Development and Family Studies or Social Work or Associate Degree in Human Services is preferred. Must possess, at minimum, a high school diploma or GED and be committed to completion of a Family Services Credential within 18 months of hire. Must possess a valid PA driver's license and an accident-free driving record.
