Job Title : Licensed Practical Nurse (LPN) - Full-Time Evenings
Job Specialty : Long-Term Care (LTC)
Job Duration : 13 weeks
Shift : 5x8.0 hour shifts

Experience : Minimum 1 year as an LPN in a Long-Term Care Facility
License : NY LPN Licensure
Certifications : Basic Life Support (BLS) through AHA or RQI
Must-Have : Weekend / Holiday rotation required
Job Description :
- Demonstrate knowledge and skills necessary to provide care appropriate to adult and geriatric patients.
- Possess the ability to assess patient data and interpret information to address age-specific needs.
- Provide direct and indirect skilled nursing care for assigned residents.
- Follow defined policies and procedures under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Maintain current knowledge and clinical expertise through continuing education and in-service programs.
- Collect data on residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to creating a developmentally age-appropriate and culturally relevant plan of resident care.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals.
- Collaborate with residents / families and members of the Interdisciplinary Team to ensure customer satisfaction.
- Participate in the mentoring / orientation of staff and consider safety, cost, and impact in care delivery.
- Provide leadership in the professional practice setting under RN direction.
- Adhere to tasks involving potential contact with blood, body fluids, or tissues, with reasonable anticipation of occupational exposure to bloodborne pathogens.
