Job Description
Job Title : LPN Full-Time
Job Specialty : Long-Term Care (LTC) LPN
Job Duration : 13 weeks
Shift : Day / Evening Rotating - 8x5
Guaranteed Hours : 36-40 hours per week (depending on shift assignment)
Experience : 1 year experience in an LPN capacity at an LTC facility
License : NY LPN Licensure
Certifications : BLS (certification through AHA or RQI specifically)
Must-Have : Availability for every other weekend and holiday shifts
Job Description :
- Provides direct and indirect skilled nursing care for assigned residents.
- Collaborates with Registered Nurses to follow defined policies and procedures.
- Maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Engages in continuing education and in-service programs to maintain a current level of knowledge and clinical expertise.
- Possesses the ability to provide care to geriatric residents.
- Collects data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicates findings to RN and other members of the Interdisciplinary Team.
- Contributes to the creation of a developmentally appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions.
- Recommends revisions to the plan of care after evaluating resident outcomes and indicates the need for referrals to ensure all aspects of the resident's care have been addressed.
- Collaborates with the resident / family and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction.
- Participates in the mentoring / orientation of staff and considers factors related to safety, cost, and impact in planning and delivery of care.
- Demonstrates knowledge and competency in nursing practice and performs tasks involving potential contact with blood, body fluids, or tissues.
