Responsibilities

Key Responsibilities:

• Develop and implement individualized care plans that address the unique needs of each patient.
• Provide high-quality patient care, including administering medications, wound care, and other medical interventions as prescribed by the physician.
• Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings.
• Conduct regular assessments of patients to evaluate their progress and adjust care plans as needed.

Requirements

Requirements:

• Current RN licensure in the state of practice.
• Current CPR certification.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
