What are the responsibilities and job description for the Food Farmacy Coordinator/Nutrition Specialist position at Hurley Practice Management Services?
GENERAL SUMMARY:
Oversees day to day operations of the Hurley Food Farmacy, which requires frequent contact with patients, volunteers, community partners, and health care providers; Provides individual/family nutrition consultations and triages patients to community resources.
SUPERVISION RECEIVED:
Reports directly to the Administrator of Wellness and Population Health Management
RESPONSIBILITIES AND DUTIES:
1. Organize the food farmacy for optimal workflow and efficiency.
2. Coordinate food orders/deliveries with Community partners (Food Bank, Flint Fresh, Edible Flint, etc.)
3. Collaborate with fellow dietitians and wellness/diabetes team to ensure proper foods are ordered/educational materials provided and available in the farmacy to support prescriptions for healthy diets.
4. Maintain temperature logs on refrigerator, freezer and other equipment in the food farmacy and ensure safe food handling procedures are in place and followed at all times.
5. Develop patient food prescription packages based on patient health and food preferences.
6. Data entry into electronic health records to accurately input data on farmacy usage/chart notes and maintain patient confidentiality at all times.
7. Maintain data/reports of utilization.
8. Provide appropriate nutrition education and counseling to patients visiting the food farmacy. Triage them into appropriate HMC and Community resources as needed.
9. Develop healthy recipes and handouts for patients.
10. Manage food farmacy volunteers
11. Work closely with HMC Marketing team on promotional materials.
12. Work closely with leadership to ensure effective and efficient program/service.
13. Provide Nutrition education/cooking demos for community events, employee wellness programs, and corporate wellness programs.
14. Represent HMC on various community projects as subject-matter expert.
15. Administer nutritional assessments for adult population.
16. Provide medical nutrition therapy/nutrition counseling via individual and group appointments.
17. Performs other related duties, as required.
MINIMUM ENTRANCE REQUIREMENTS:
- Registered dietitian (credentialed with the Commission on Dietetic Registration for the American Dietetic Association) or registry eligible to become a registered dietitian within nine (9) months from date of appointment to this position or the employee may be terminated.
- Ability to communicate effectively both orally and in writing.
- Ability to speak in front of large groups.
- Ability to maintain accurate and current reports and records.
- Ability to establish and maintain effective and harmonious working relationships with team members and patrons.
Job Type: Full-time
Pay: $55,000.00 - $70,000.00 per year
