As a direct service provider of community services the Case Manager/Intensive
Individual Support is accountable for delivering interactive mental health and applied behavior analysis services in the community, school or home setting to children designated as having significant mental health and social challenges. The goal of the Case Manager/Intensive Individual Support is to provide evidenced based services and improving a client’s emotional, mental, and overall functioning while maintaining the child/youth in the least restrictive environment of their home, school, and community

ESSENTIAL FUNCTIONS:
- Provides direct services to accomplish skill training on target goals and objectives using evidence-based practices with measurable outcomes, with measurements as determined by medical necessity by the Qualified Mental Health Professional and/or BCBA/Autism Specialist.
- Follow treatment interventions as outlined by BCBA/Autism Specialist.
- Regularly attend supervision meetings and trainings as required by BCBA/Autism Specialist.
- Deliver intensive, community-based, interactive mental health and applied behavior services to children and adults utilizing a strengths-based, skill-building philosophy, and evidence-based practices.
- Participate in the development and maintenance of a client-centered Treatment Plan along with the family and treatment team.
- Assist the youth and/or family to , or adult, develop a basic understanding of their symptoms and possible solutions.
- Assist the adult, or youth, and/or family to enlist utilize natural community supports in developing solutions.
- Assist in coordination of mental/physical healthcare services and facilitate communication between the client, service providers, family and community partners.
- Other duties as assigned.
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
- Strong organizational skills.

- Intermediate proficiency with Microsoft Office (including Microsoft Word and Microsoft Excel)

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

One of the following is required:
- Bachelor’s degree, preferably in Social Work, Psychology, or some related field OR
- Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities OR
- Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years
PREFERRED EDUCATION AND EXPERIENCE:
- Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics)
- Three to five years’ professional experience in mental health services or a related field
REQUIRED LICENSE/REGISTRATION/CERTIFICATION:
- Employee must complete CPST Case Management on-line training, KHS on-line training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Within 60 days of employment, the employee must have completed Adult state required mental health Ccase mManagement training.
- Employee must complete state required training to become an Intensive Individual Support within 6 months of hire date.
- Valid driver’s license, insurance, and a dependable vehicle
