CBS Access Specialist - Horizons

The CSS Access Specialist provides a crucial role in the Community Support Services (CSS) Department in helping ensure that all clients who are referred for CSS services are promptly engaged in those services. The Access Specialist also provides a high level of organization to the department to ensure that all needed client documents and assessments are completed by staff in a timely manner and that client care is not impeded due to administrative tasks being incomplete. The Access Specialist will provide introductory care to clients who are referred for CSS services and provide a comprehensive education to clients about the services that they are entering into as well as serving as a ‘face of the Department’.

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
ESSENTIAL FUNCTIONS:
  • Builds effective, professional relationships by working in partnership with consumers and families while emphasizing consumer strengths, choice and empowerment
  • Is the initial point of contact for clients who are referred for CSS services and provides the initial service delivery for all CSS referrals.
  • Helps make sure that when clients are referred for CSS services that they are able to begin engaging in services within the first 10 days after referral.
  • Utilizes a trauma-informed, person-centered approach to work with diverse populations.
  • Tracks the due dates of data centered assessments and documents that need to be completed by clients/staff. Also helps staff stay oriented to when those documents need to be completed.
  • Displays sound judgement in crisis, ongoing treatment support and in all facets of the job through respecting ethical boundaries and confidentiality
  • Contributes to data collection and entry for the CSS department to help with process and quality improvement.
  • Coordinates with internal and external providers to ensure quality care is received by consumers
  • Utilizes constructive communication skills in person, over the phone and electronically to coordinate care
  • Engages in positive working relationships with within the agency and with outside agencies
  • Completes all assigned trainings by established deadline
  • Completes related activities as assigned by the manager/director
  • Other duties as assigned.
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
  • Basic familiarity with community mental health services in general and services within Horizons Mental Health Center preferred.
  • Basic familiarity with Microsoft Office
PREFERRED KNOWLEDGE AND SKILLS REQUIRED:
  • One to three years’ experience working in a behavioral health setting.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
  • One of the following is required:
    • Bachelor’s degree, preferably in Social Work, Psychology, or some related field OR
    • Associate's degree with an additional 2 or more years' experience working with educating and teaching consumers about self-care, improving daily living activities OR
    • Total combined years of schooling and experience (working with educating and teaching consumers about self-care, improving daily living activities) to equal 4 or more years
PREFERRED EDUCATION AND EXPERIENCE:
  • Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics)
  • Three to five years’ professional experience in mental health services or a related field
  • Self-identify as a present or former primary consumer of mental health services
REQUIRED LICENSE/REGISTRATION/CERTIFICATION:
  • Within 30 days of employment the employee must have completed CPST Case Management on-line training, KHS on-line training. Within 60 days of employment, the employee must have completed Adult Case Management training.

  • Valid driver’s license, insurance, and a dependable vehicle
