What are the responsibilities and job description for the Supports Coordinator - ID (Intellectual Disabilities) position at HVHS Brand?
Department: Stanton Clinic
Work Hours: Monday through Friday 8:30 AM until 5 PM.
To provide professional social work, education and advocacy to individuals with intellectual disabilities and their families; to locate, coordinate and monitor needed services and supports, to access quality services and supports so that individuals with intellectual disabilities may live self-determined lives, and to be accepted as valuable members of the community.
Requirements:
Bachelors Degree in Psychology, Social Work, Counseling or related field with three years experience working directly with individuals in ID area. Valid Pennsylvania Drivers license. Basic cell phone, computer and keyboard skills, including familiarity with Microsoft Office (Word, Excel and Outlook) and the internet.
Successful completion of Acts 73 and / or 169 clearances upon commencing employment, if applicable.
Preferred:
ID Supports Coordination experience. Extensive knowledge of persons with disabilities, knowledge of regulatory agencies and payor policies, as well as community resources. Knowledge of persons with disabilities and of regulatory agencies and payor policies, as well as community resources.
