Standard Requirements
- Support the Missions, Values, and Vision of Hyatt Family Facilities and the facility.
- Protect Resident Rights and ensure an atmosphere which allows for the privacy, dignity and well -being of all residents in a safe, secure environment.
- Support, coordinate with, and implement specific procedures and programs for: Safety, including universal precautions and safe work practices, established fire/safety disaster plans, risk management and security, report and/or correcting unsafe working conditions, equipment repair and maintenance needs
- Confidentiality of all data, including resident, employee and operations data.
- Support and participate in common teamwork Cooperates and works together with all co-workers; plan and completes job duties with minimal supervisory direction, including appropriate judgment.
- Uses tactful, appropriate communications in sensitive and emotional situations.
- Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems, and concerns.
- Promotes customer service with residents, family members and guests.
- Adheres to requirements for in-service training; attendance, uniform, department and dress code policies, including personal hygiene, and completes other work duties as assigned.
DUTIES:
- Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in the planning of the resident's admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with hospice, podiatry, audiology, and dental companies
- Documentation in the resident’s clinical record
REQUIREMENTS:
- EXPERIENCE WITH MDS 3.0 or PDPM preferred
- Preferred Bachelor's Degree in related area
- Long term care/ sub-acute experience is a plus
