Job Overview
Coordinate services offered by Social Services Department and promote the psychosocial growth and well-being of residents, family and staff. Assist residents and their families in finding and utilizing financial, legal, mental health, transportation and other community resources.
Standard Requirements

3. Support, coordinate with, and implement specific procedures and programs for:
a. Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management and security, report and/or correcting unsafe working conditions, equipment repair and maintenance needs
b. Confidentiality of all data, including resident, employee and operations data.
4. Support and participate in common teamwork:
a. Cooperates and works together with all co-workers; plan and completes job duties with minimal supervisory direction, including appropriate judgment.
b. Uses tactful, appropriate communications in sensitive and emotional situations.
c. Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems, and concerns.
d. Promotes customer service with residents, family members and guests.
e. Adheres to requirements for in-service training; attendance, uniform, department and dress code policies, including personal hygiene, and completes other work duties as assigned.

Essential Duties
1. Provide information for residents and families concerning POLST forms and living wills.
2. Provide in-service training and consultation for all staff concerning psychosocial needs of families and residents. Promote psychosocial growth and well-being in residents, family and staff.
3. Responsible for all AIMS testing per OBRA regulations as well as consultation with other care providers.
4. Monitor psychotropic medications and behavioral flow sheets. Facilitate Gradual Dose Reductions, where appropriate with consultation from Nursing Services.
5. Coordinate mental health and psychiatric services for residents as needed.
6. Obtain residents social history and update as necessary in residents medical record.
7. Participate with interdisciplinary team in developing individual resident care plans.
8. In coordination with other disciplines, develop a comprehensive discharge plan and assist family and resident with arrangements.
9. Arrange and conduct care conferences.
10. Meet with new residents and family members to orient them to facility procedures and complete initial MDS assessment. Coordinate facility services offered.
11. Provide counseling to residents and residents family members in times of need.
12. Advocate for the protection of residents rights and responsibilities.
13. Complete and update resident Personal Belongings sheets as needed.
14. Facilitate the resolution of resident and family concerns.
15. Train, supervise and complete annual reviews of Social Services Staff.
16. Other duties as assigned by administrator.

Physical Demands of the Job:
1. Frequent tasks involving walking throughout the shift. Distances typically less than 200 feet with occasional walking greater distance, walking is on even surfaces.
2. Occasional sitting during duties, the majority of which will be at your workstation in ergonomically correct chair and desk.
3. Occasional standing does not need to be static.
4. Occasional squatting and/or bending, necessary when obtaining paperwork.
5. Lifting up to 10 lbs. frequently, 11 to 25 lbs. occasionally, 26 to 50 lbs. seldom and lifting greater than 50 lbs. is not required.
6. Repetitive movements requires following proper body mechanics. Notify supervisor of any concerns. Fine repetitive motor skills will be used frequently; gross repetitive motor skills will be used occasionally.
7. Occasionally push/pull up to 25 pounds of force.
8. Frequently reach from shoulder height to knees; seldom reach above shoulders or below knees.
Conditions of Environment
1. Controlled temperature, primarily indoor, well lit, maintained flooring, smoke free and occasional odors that dissipate. Noise level is maintained quiet to moderate.
Basic Qualification
Education/Training: High school diploma or equivalent required. BA in Social Work or Social Services Consultant is required for 120 or more bed facility per WAC 388-97-0960.
Skills: Ability to deal tactfully with co-workers, residents, family members, visitors and the general pub-lic. Must have the ability to work with ill, disabled, elderly, emotionally upset, and, at times, hostile peo-ple within the facility while remaining calm and professional. Must have strong communication skills and an ability to listen to and empathize with others.
Experience: Long term care experience or health care social work preferred. Supervisory experience pre-ferred.
