Summary/Objective
This position will provide program leadership and direction for the Community Integration Services division- for adults with I/DD and complex behavioral health needs. This positions will work closely with program team from adult day services as well as clinical teams from behavioral health.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Represent Boundless at Intake, Individual Service Plans (ISPs), and Team Meetings.
Train management and staff on ISPs and pertinent information shared at meetings.
Ensure implementation of all sections of ISP for both DODD and ODMHAS.
Serve as a liaison with Support Administrators and families.
Oversee development, revision, and quality control of service delivery documentation.
Conduct monthly outcome and service delivery documentation reviews.
Ensure legal compliance with DODD and ODMHAS regulations.
Document, review and report to the required entities any incidents adversely affecting the health and safety of an individual served.
Ensure standards of CQL are maintained and exceeded.
Lead the general referral, admission and transition of persons served into and within the scope of services offered at Boundless.
Complete intakes, tours, assessments for individuals served to ensure person centered service deliver and whole person care.
Attend staff meetings, and other meetings, as assigned.
Complete all required documentation related to funding, employment, or general operations.
Assure that proper procedures are followed regarding individuals and their inherent rights.
Assist in the timely submission of all records, files, financial records, etc.
Complete weekly reporting for key performance indicators and implement plans to correct areas that are not meeting sustainability models.
Ensure a healthy and safe environment for both individuals and staff members.
Assist clients with developmental disabilities in attaining the optimal level of independence.

Required Education and Experience
High School Diploma or equivalent
Three years experience in program management and supervision in the developmental disabilities field, preferably in day habilitation services.
Valid Driver’s License and Insurable Driving Record.
Demonstration of excellent oral and written/ documentation skills with strong teamwork skills.
Ability to pass a background and drug screening meeting Ohio Adm. Code 5101:3-45-11 for Disqualifying Offenses.
Positive attitude and enthusiasm for providing high-quality services to individuals with developmental disabilities.
Preferred Education and Experience
Five to 10 years experience in program management and supervision in the developmental disabilities field, preferably in day habilitation services.
Preferred Bachelor’s degree in Human Services from an accredited College or University.
Additional Eligibility Qualifications
Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with consumers and other staff.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
Must possess the ability to make independent decisions when circumstances warrant such action.

This job operates in an office environment, but with travel and community engagement as needed.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee is required to lift, bend, push, pull up to 50lbs.
Position Type and Expected Hours of Work
This is a full-time, exempt position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work may be required as job duties demand.
