SHIFT : 8am-5pm (M-F)
JOB TYPE : Hybrid-Full-Time Permanent

We're seeking candidates who are passionate about providing top-quality care and are dedicated to expanding access to comprehensive healthcare. If you're enthusiastic about establishing personal connections, providing exceptional patient education, and embracing a holistic approach to care, you are the ideal candidate to embody our values and make a difference in every patient's life. This role can include in-clinic, in-home, or telehealth visits. Home visits will be scheduled within a 30 mile radius of your home region. This position will be required to flex across regions to provide coverage as needed to accommodate for scheduling changes.
Responsibilities :
- Conducting annual wellness visits primarily at the patient's residence, supplemented by telehealth
- Thoroughly documenting medical histories, symptoms, and medication information
- Administering various tests and screenings
- Collaborating with our Primary Care team to ensure holistic patient care
- Ensuring timely completion of patient records, with access to electronic medical records and portals
- Providing patient education on preventive care and prescribed treatments
- Maintaining strict confidentiality in line with HIPAA regulations
