What are the responsibilities and job description for the Case Manager position at IBA Support Services?
A Full-Time Qualified Intellectual Disability Professional Case Manager, providing Service Facilitation/Self Directed Assistance in the Home-Based Support Services program. Essential characteristics of self-motivation, flexibility and a concierge outlook to provide the highest quality services for our clients.
Hours: Monday-Friday 9:00 a.m. to 4:00 p.m. Daily schedule flexibility is required including afternoon, evening and some weekend availability. Travel is required between two office locations
Responsibilities and Duties
- Manage a caseload of adults with intellectual disabilities in the home-based program.
- Collaborate with client’s support team members and facilitate team meetings.
- Construction of the implementation strategy plan, ongoing monitoring and implementation of services to ensure consistency and continuity of care.
- Conduct assessments, create and identify needs, conduct research and coordinate services.
- A considerable amount of advocacy, documentation and communication with clients, families and providers.
- Maintain client files in accordance with DODD, Medicaid, and agency requirements.
- Maximize available billable hours and complete and submit billing documentation on time.
- Other duties as assigned.
Qualifications and Skills
- Associates or Bachelor’s Degree in a human services-related program (Required).
- Minimum of 1 year experience working with children and/or adults with developmental disabilities (Required)
- Must possess the following knowledge, skills, and abilities of a DD/ID case manager including:
- Skills in data collection and interviewing
- Oral and written communication skills
- Knowledge of developmental disabilities and mental illness
- Knowledge of treatment planning, decision making, and needs assessment
- Willingness to travel often and work in client’s home environment (Required).
- Valid Ohio Driver’s License (Required)
- Oral and written communication skills
- Experience working with the Department of Developmental Disabilities, (Preferred).
