We are looking for a flexible for shift Case Manager to work in out Mount Vernon Service Center!
Position Qualifications[1]:
· High School Diploma or GED Equivalent
· Valid Driver’s License
· Reliable transportation and current automobile insurance
· Successful completion of all agency required background checks
· Knowledge of applicable laws, codes and regulations
· Microsoft Word Products and any computer applications utilized by RRS, LLC
· Ability to operate standard office equipment (i.e., printer, computer, etc.)
· Effective Communication Skills
Job Duties:
· Responsible for providing community based services that encourage appropriate multi-system involvement and to ensure continuity and effectiveness of services and/or systems to the client;
· Educate clients or community members about mental or physical illness, abuse, medication, or available community resources;
· Assist clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing support;
· Provides direct clinical care;
· Refer patient, client, or family to community resources for housing or treatment to assist in recovery from mental or physical illness, following through to ensure service efficacy;
· Communicate with families, probation officers, caseworkers, schools, and other interested parties in order to exchange necessary information during the treatment process and documents all client- related contact in compliance with agency standards;
· Maintains a complete and updated clinical record for each assigned client; utilizes the agency’s established standard operating procedure to comply with documentation requirements;
· Engages in supervision, consultation, and applicable educational opportunities to develop expertise in field of service;
· Participates in peer review process; conducts regular case compliance reviews of charts;
· Communicates effectively, both verbally and in writing. Shares information and demonstrates ability to listen openly and empathetically to others;
· Works collaboratively with fellow employees and others to achieve an identifies goals and objectives;
· Contributes to a positive work environment by treating others with fairness and respect;
· Demonstrates the ability to pro-actively plan work and proceed with a task with limited instruction. Willingly steps up to take on new responsibilities or to help a co-worker complete a task;
· Supports the relationship between the agency and the community by demonstrating respectful, courteous, and cooperative behavior when interacting with clients, visitors, and other employees;
