TITLE : Family Education Facilitator

QUALIFICATIONS :

  • Minimum of Bachelor's Degree in Social Work, Counseling, Psychology, Child Welfare, Human Services, Family Services, or other fields closely related to providing social services to families of young children.
  • Possess proficient oral and written communication skills.
  • Physical and mental stamina and ability to perform job functions, tasks, and duties.

REPORTS TO : Family Services Coordinator

SUPERVISES : Non-Applicable

TERMS OF

EMPLOYMENT : 12 Months

PAY GRADE : 24

JOB GOAL : To provide support services for families that will lead to student achieving academic success.

PERFORMANCE RESPONSIBILITIES :

  • Full enrollment and a waiting list are met in a timely manner.
  • Assigns proper funding codes upon approval from Family Services Coordinator.
  • Informs all necessary parties of any funding code and transportation changes in a timely manner.
  • Is readily available in the event a student from school site is undeliverable.
  • Implements all Early Childhood transportation policies and procedures consistently.
  • Assists the Family Services Coordinator with duties required to fulfill the Early Childhood Program Performance Standards.
  • Completes and implements Family Partnership Agreements.
  • Updates and reviews Family Partnership Agreements and make necessary referrals.
  • Works in continual collaboration with other program systems to ensure that supportive services necessary for family self-sufficiency are available and all program standards are met.
  • Adheres to case management model and procedures including, timely and accurate documentation, confidentiality, completion of strengths and needs assessments, individualized family partnership agreements, and timely follow-up.
  • Maintains accurate eligibility / income documents.
  • Complete necessary updates and transfer student documents to ensure smooth transitions.
  • Provides a seamless transition in and out of the Early Childhood program for children and families.
  • Provides opportunities for parents to participate in the program as committee members, policy council representatives and volunteers.
  • Provides opportunities for parents to enhance their parenting skills and knowledge of developmental needs of their children through trainings, meetings, and community links.
  • Maintains confidentiality.
  • Assists with the recruitment and enrollment of children into the Early Childhood Program in order to ensure full enrollment and a waiting list are met in a timely manner.
  • Attends recommended in-services, workshops, trainings, and meetings.
  • Provides opportunities for parents to enhance their knowledge of child development through conducting workshops in collaboration with the Early Childhood Teacher, School Administrator, and or Family Services Coordinator.
  • Conducts regularly scheduled meetings with the Early Childhood teacher, school nurse, and school administrator to discuss relevant information concerning support for Early Childhood children and families.
  • Conducts home visits.
  • Perform any additional duties assigned by the superintendent and / or evaluator.
  • PROFESSIONAL ETHICS :

  • Displays professional ethics on and off duty.
  • Adheres to all Iberville Parish School Board's Policies
  • Reports job-related injuries within 24 hours of work accident to the immediate supervisor.
  • Assumes responsibility for implementation of emergency procedures.
  • Seeks and maintains a positive attitude of promoting school and community relations.
  • Participates in and promotes harmonious working relationships with students, colleagues, parents and community members.
  • Attends meetings as directed.
  • Serves on task force and decision-making committees, when appropriate.
