What are the responsibilities and job description for the System of Care Outreach Coordinator position at ICAN Inc?
The System of Care Outreach Coordinator would serve as the point person to school districts, community agencies, and referral partners throughout the county. This position will collaborate with key stakeholders and support them in identifying youth and families that would benefit from mental health services, and help to oversee the referral linkages to those services. This position will further support the SOC initiative through assisting Child and Family team with data collection, and the facilitation of Girls Circle as needed. The Outreach Coordinator adheres to and promotes the philosophy and missions of the company by performing the following duties and responsibilities.
Duties and Responsibilities:
- Act as the main point of contact for referring agencies or staff, for purposes of ensuring seamless movement of clients through the System of Care services and supports.
- Assign incoming inquiries to the Support Coordinator and/or Family Service Coordinator, or redirect referrals to the most appropriate linkage resources.
- Share critical information and updates regarding SOC services and referral processes to Project Director.
- Maintain a master roster of youth and families that enter System of Care services and report data to the SOC Project Director for evaluation and SAMHSA data collection.
- Utilize the agency's electronic health record to enter documentation, data collected, and other pertinent client information.
- Support SOC Child and Family team by completing program intakes, reassessments, and participant follow-ups as needed.
- Complete data collection tool requirements in accordance with the agency and SAMHSA, including but not limited to: National Outcome Measures (NOMs), Strengths and Difficulties Questionnaire (SDQ), Ask Suicide-Screening Questions (ASQ), Child Trauma Screen (CTS), and Results Based Accountability (RBA).
- Complete training in evidence-based programs, such as Girls Circle, and collaborate with other SOC partners on implementation of program(s) according to grant guidelines and overview.
- Participate in regular community meetings as assigned, including, but not limited to, Youth Services Task Force, Suicide Prevention Coalition, Resource Sharing Network, etc.
- Conduct community outreach on a regular basis, typically occurring during evenings and/or on weekends, and take point on coordinating efforts that will draw attention to SOC and help to increase the number of referrals.
- Monitor and maintain engaging SOC social media content.
- Additional responsibilities may be assigned on an as needed basis.
Education/Experience:
- Bachelor’s Degree in Psychology, Social Work, Human Services, or related field preferred
- Two years relevant experience in traditional and non-traditional youth service settings.
- NYS Drivers License is required
