JOB SUMMARY:
The position is responsible for the daily assistance of the Day Program. Ensure that all contractual and programmatic requirements are met consistently and will address and resolve all concerns within the program in an efficient and timely manner.
Essential Job Duties include, but are not limited to:
- Carry out agency policies and procedures to ensure the overall quality of the programs offered at iCare Health Services (iCHS), LLC.
- Be part of the team in the day-to-day operations of the Day programs to include paperwork requirements, management and ensure that contractual and programmatic requirements are met consistently.
- Implementing and monitoring Behavior Treatment Plans as required by the Division of Developmental Disabilities.
- Assist with daily activities, living skills, behavior management, and help members achieve their goals stated in their ISP.
- Assist with field trips and outings. Transportation of members required.
- Assist in developing, implementing, documenting and monitoring of each individual goals for the Members attending the learning center.
- Assist individuals with personal care tasks; such as eating, toileting, and other daily living activities while at the center.
- Teaching basic independent living skills according to their ISP such as household tasks; such as cleaning, cooking, etc.
- Assist in the development of the monthly calendar.
- Maintain weekly case notes and daily objective sheets tracking ISP and/or BTP goals for participants
- Exhibit appropriate role model behavior by using appropriate language, dress, manners, and respect.
- Ensure a healthy, clean and safe program environment
Additional duties: Medication administration, supervising members, provide emotional support, and having fun!
- Participate in meetings, trainings, planning functions and committee memberships as assigned.
- Support and advocate for the individuals and families we serve.
- Work in partnership to develop and maintain positive, professional relationships with our members, providers, and representatives from the Division of Developmental Disabilities.
Skills required for the position:
- Have at least three (3) months experience in conducting group or individual activities related to specific developmental, habilitative, or recreational programs.
- Experience working with the Division of Developmental Disabilities.
- Experience supporting and interacting with individuals with developmental disabilities.
- The ability to work in partnership with other professionals, including case managers, medical staff, therapists, etc.
- Excellent verbal and written communication skills.
- Excellent organization skills and the ability to multi-task.
- Must have the ability to work independently and be self-directed.
- Must be able to provide guidance and support to others.
- Must maintain professionalism at all times while representing the company, and also maintain a professional demeanor and appearance.
- Must possess the ability to work well under pressure and handle conflict in a professional and efficient manner.
- Ability to balance, bend, stand, walk, kneel, twist, stretch, climb and sit.
- Ability to lift/ carry up and push/pull to 50 pounds unassisted.
- Ability to tolerate environmental conditions, such as hot, cold, wetness, dust, noise, etc.
- Ability to drive and travel independently.
- Ability to communicate effectively.
- Ability to follow instructions.
- Ability to work effectively with staff, clients, and the public.
- Ability to use judgment.
LICENSE/CERTIFICATION NEEDED:
Ability to satisfactorily complete all necessary in-service training, and have your CPR and basic first aid, Article 9, DCW (obtained within 90 days of hire), Fingerprint card, Prevention /Support and medication administration certification (if Applicable);
