Job Duties & Responsibilities :

• Represent LEAP in the community at large to promote the services of the program.
• Support planning, implementation, and coordination of the program and its activities.
• Ensure consistent and complete documentation of program services and activities.
• Support maintaining program related partnerships with community-based and neighborhood organizations.
• Coordinate program communications internally and externally.
• Support the Social Worker Team to ensure that high-quality services are consistently delivered and the records are maintained in accordance with appropriate standards.
• Handle confidential information and documents, as well as potential crises.
• Attend staff meetings, training, and other meetings as required.
• Travel to and from home sites when needed.
• Other duties as assigned.
