The Nurse Practitioner plays a vital role in our Advanced Illness Management team, providing specialized care to patients with complex medical needs. This role involves comprehensive patient assessments, individualized treatment plans, and ongoing communication with care teams to enhance patient quality of life.
About the Role:
- Work collaboratively with physicians and other specialists to deliver advanced illness management and palliative care services.
- Conduct thorough patient assessments, establish individualized treatment plans, and communicate effectively with care teams.
- Prioritize patient needs, provide high-touch care, and address symptoms and stress associated with illness.
Key Qualifications:
- Masters of Science in Nursing from an accredited school of nursing or equivalent experience.
- Minimum of two years of Nurse Practitioner experience, preferably in Family or Adult Medicine.
- Certification in Hospice and Palliative Care (ACHPN) is preferred.
