- This is a full-time role where you will work collaboratively with the medical staff. As an Advanced Practice Registered Nurse (APRN), you will play a vital role in managing the medical needs of residents in skilled nursing facilities. - You will deliver primary care, urgent care, and preventative care services to residents in the assigned skilled nursing facility. You may also provide care to residents in assisted living facilities.
- You will verify admission orders and conduct initial assessments of residents. This includes performing physical exams, reviewing medications, and screening for depression.
- You will collaborate with the facility's medical director, attending physicians, and nursing staff to provide the highest level of care to patients.
- You will communicate with and counsel family members on treatment options when there are significant changes in a resident's condition.
Minimum Qualifications for Nurse Practitioners:
- Valid Nurse Practitioner License in the state of Kentucky
- DEA License
- National certification as a Nurse Practitioner
- Clinical background in adult, family, or geriatric specialties
- Vaccination against COVID-19 is mandatory for all new hires, unless eligible for a remote, religious, or medical exemption.
Preferred Qualifications for Nurse Practitioners:
- Experience in internal medicine, gerontology, or cardiology
- Excellent administrative, organizational, and verbal skills
- Ability to work independently

- Dependable and reliable
- Prior nursing experience in long-term care/skilled nursing facility is a plus
