We are seeking a motivated and clinically skilled Pharmacist to join our pharmacy team. This full-time position offers a dynamic mix of clinical and distribution responsibilities, with clinical work making up more than 60% of the monthly schedule. As part of our collaborative healthcare environment, you will provide pharmaceutical care to diverse patient populations across inpatient settings.
Key Responsibilities:
This non-profit health system is the largest employer in northeast Indiana and northwest Ohio, offering care across 12 hospital campuses, multiple surgery centers, and 600 employed providers—all connected via a single Epic® platform. The flagship medical center is a 511-bed tertiary facility and Level II Trauma Center, serving patients across Indiana, Ohio, and Michigan. Specialty services include stroke, cardiac, orthopedic, oncology, and women’s and children’s care.
The pharmacy department is known for its innovative technology adoption and has led the way in implementing advanced systems such as Epic®, Pyxis® MedStations™, Smart Infusion technology, and automated TPN compounders. Key Responsibilities:
- Participate in multidisciplinary rounding, seven days per week
- Oversee, monitor, prepare, and dispense medications safely and accurately
- Provide drug information to physicians and staff; ensure optimal medication use
- Monitor therapies such as anticoagulants, antimicrobials (e.g., vancomycin), TPN, PCA, and others
- Maintain accurate documentation of interventions and patient outcomes
- Mentor and precept IPPE/APPE pharmacy students and pharmacy residents (PGY1 & PGY2)
- Support safe medication practices, regulatory compliance, and hospital protocols
- Doctor of Pharmacy degree (PharmD) from an ACPE-accredited institution
- Completion of PGY1 pharmacy residency or equivalent hospital practice experience
- Eligible for licensure as a pharmacist in the state of Indiana
- Strong clinical skills and a passion for patient-centered care
- Excellent communication and teaching abilities
