What are the responsibilities and job description for the Community Health Navigator position at Imagine Pediatrics?
What You’ll Do
As part of our Transitions of Care Team, this role assists patients in getting post-hospital care in the form of telephonic and in-person patient support.
- Work as part of an interdisciplinary, collaborative team to ensure we are meeting patient needs
- Assist patients in enrolling in Imagine Pediatric programs
- Round at facilities in your geographic area to meet with Imagine Pediatric patients and caregivers
- Coordinate with facility discharge planners to ensure that there are no barriers to patients obtaining after-hospital resources and care
- Attend daily huddles with your team to communicate patient needs and progress
- Liaison with hospital and other health systems to make staff aware of Imagine Pediatric program and support
- Supporting patients with any in person/community needs related to care coordination/discharge support. These could include, but are not limited to, pharmacy fills, PCP coordination, SDOH needs, and home visits.
What You Bring & How You Qualify

- Bachelor’s degree required
- Bilingual (Spanish) highly preferred
- Texas or National CHN certification (obtained within 6 months of hire date)
- CPR certified
- 3 years of experience in a healthcare setting
- Ability to adapt to changes
- Ability to self-start and self-structure
- Warm bedside manner
- Excellent written and oral communication skills
- Strong critical thinking skills
- Ability to manage multiple tech platforms
- Experience working in a patient-facing hospital or facility role
- Experience working as part of an interdisciplinary team
- Ability to organize and prioritize daily schedule
- Valid Texas driver’s license
