Job Summary:
Independent Nursing Care (INC) is seeking a compassionate and detail-oriented Nurse Assessor to join our Home and Concierge Care Departments. This flexible, hybrid, part-time position focuses solely on the nursing aspects of client care and staff support, with the opportunity to transition into a full-time role within the company.
Responsibilities include conducting in-home assessments, developing personalized care plans, coordinating services, and ensuring high-quality care for a manageable number of clients. The Nurse Assessor will receive strong support from the Director, allowing them to focus entirely on delivering exceptional clinical care. Communication with management, employees, supervisors, and clients is essential, along with a thorough understanding of company policies, procedures, and work processes. This role is ideal for a dedicated nursing professional passionate about helping individuals remain safe and independent at home.
Supervisory Responsibilities:
Field Staff Employees: Home Health Aides, Companion Caregivers, RN’s, LPN’s
Responsibilities:
- Perform in-home assessments and visits to evaluate client needs and develop care plans.
- Assist with coordinating care and services with clients, families, and healthcare providers.
- Monitor client progress and adjust care plans as needed.
- Ensure compliance with regulatory requirements and best practices.
- Communicate with clients, employees, and families to ensure quality care, document, address clinical inquiries, follow up as needed, and notify field staff of client condition changes.
- Conduct staff supervisory visits, provide reports, review documentation, monitor charting deficiencies, and ensure timely follow-ups to support quality care.
- Potential to assist with nursing clinical home visits for wound care, immunizations, blood draws, IV infusion, medication management, or other visits as needed.
Qualifications & Requirements:
- Active Registered Nurse (RN) license in New York State (required).
- CPR Certification
- Minimum of 3 years of experience in home health care, case management, or assessments preferred.
- Strong knowledge of NYS LHCSA regulations, DOH requirements, and home care standards.
- Excellent communication, presentation, and leadership skills.
- Proficiency in Microsoft Office and learning management systems (LMS) is a plus.
- Strong organizational and interpersonal skills.
- Ability to work independently and travel within the service area.
- Ability to handle sensitive and private information with discretion.
- Friendly, caring, professional demeanor.
