POSITION SUMMARY: Registered Nurse (RN) - Nurse Manager is a baccalaureate prepared Registered Nurse who provides comprehensive nursing services to urban Native American individuals and families whom reside in the greater Phoenix metropolitan area as well as the uninsured and underinsured populations. Nurse management services include health assessment, physical examination, health education, counseling, communicable disease intervention, psycho-social evaluation; perform skilled nursing procedures, referral to identified resources and services, case management, anticipatory guidance and conducting Health Education classes in the home and clinic settings. The Nurse Manager supervises the Medical Assistants (MA) and Clinical Care Coordinator, case manages the prenatal patients, serves as the Employee Health nurse, assists with infection control, oversees the Vaccine for Children (VFC) program, laboratory and serves as consultant to the Health Start program. The RN-Nurse Manager will represent the company professionally and positively to enhance and promote the core values and mission of the organization, always exercising utmost discretion, diplomacy and tact in patient/staff interactions. The Nurse Manger will report directly to the Medical Director. Duties: RESPONSIBILITIES AND DUTIES: Duties may include, but are not limited to the following: 1. Demonstrates excellent patient relations skills on the phone and in-person, e.g. smiles, cheerfully greet patients, engages all patients in a genuine, warm and caring manner 2. Assesses patients’ health status through utilization of the nursing process, physical examination, history taking, screening tests, and observation. Provides a complete, thorough, and written assessment of the patient on arrival utilizing the nursing processes. 3. Provides direct supervision of the medical assistant staff, Clinical Care Coordinator, students and volunteers at NATIVE HEALTH Central (NHC) clinic. Maintains time and attendance utilizing computerized ADP EZ Labor System. Handles recruitment, training, retention and disciplinary action. In addition, conducts performance evaluations of staff in a timely manner. 4. Continuously collects data about the healthcare status of the patient/family in a systematic way to determine the need and type of care and treatment to be provided and the need for further assessments, using the iCare program through EHR. 5. Informs the Medical Practitioner of changes in the patient’s condition, medication and diagnostic results. Maintains statistical records and submits reports as requested. Keeps appropriate records of statistical data required by the agency for reporting purposes. 6. Coordinates Immunizations for NHC clinic - orders vaccine supplies and maintains inventory control. Responsible for reporting immunization usage to Arizona state, Maricopa County and Federal agencies. Educates and monitors Medical Assistant competency with vaccine usage. 7. Provides prenatal case management. Collects data needed for GPRA and UDS reports concerning prenatal patients. 8. Ensures the coordination and collaboration with clinical team members, patients and families to facilitate unmet health care needs within the context of the Patient Centered Medical Home (PCMH) model with the Clinical Care Coordinator. 9. Orders medical and laboratory supplies and maintains inventory control and ensures all exam rooms are properly stocked with needed supplies. 10. Functions as NATIVE HEALTH Central Infection Control Nurse (ICP). Maintains a clean, neat, and safe work environment. 11. Functions as NHC Employee Health Nurse. Keeps employee records and monitors employee’s compliance with required screening. 12. Participates in Health Start patient care conferences. Provides RN consult services to the Health Start program as necessary. 13. May serve on NATIVE HEALTH teams, wherever and whenever management determines the skills and talents of the incumbent would be best utilized including but not limited to, the Improving Patient Care (IPC), Patient-Centered Medical Home (PCMH) and the Quality Improvement committees. 14. Participates in performance improvement activities. 15. Perform other work related duties as assigned in accordance with the Arizona State Board of Nursing scope of practice for Registered Nurses. 16. Must exercise utmost diplomacy and tact to provide excellent customer service for patient; practices confidentiality and privacy protocols in accordance with NATIVE HEALTH policies and HIPAA requirements KNOWLEDGE, SKILLS AND ABILITIES: 1. Knowledge of Public Health Nursing Practice, OB, Peds, and Diabetes. 2. Ability to perform nursing care procedures and techniques pertinent to ambulatory outpatient clinic. 3. Ability to make independent judgments and work autonomously. 4. Ability to communicate with patients and staff of different economic, cultural, ethnic and educational backgrounds. 5. Able to assess physical, psychosocial, nutritional and community health needs. 6. Work effectively with multi-problem at risk individuals, utilizing problem solving skills. 7. Ability to plan and schedule daily activities for team, instruct staff on organizational skills. 8. Demonstrates strong oral and written communication skills. 9. Knowledge of case management skills. 10. Skills in using medical database, spreadsheets, RPMS/EHR and other Microsoft Office computer software systems. 11. Knowledge of community resources. 12. Act as a liaison between NATIVE HEALTH and external agencies. 13. Collaborative health planning with multi-disciplinary teams following the Medical Home model. Qualifications: QUALIFICATIONS AND EXPERIENCE: 1. Current Registered Nurse (RN) License to practice in the State of Arizona. 2. Bachelor of Science degree in Nursing (BSN) preferred. Nurse Manager, Position Description 3 3. Two years of Community Health Nursing experience and a minimum of 2-3 years of nursing experience in an acute care clinic or hospital environment. 4. Advanced computer skills including knowledge of software applications such as MS Word, EXCEL, PowerPoint, ACCESS, and be knowledgeable in the use of the Internet and email systems. 5. Must be dependable, punctual, relate well with others and able to meet deadlines. 6. Current CPR Basic Life Support Provider card. 7. Possess current and valid Arizona Driver’s License with no DUI/DWI or reckless driving convictions in the last five years having no more than two at-fault accidents in the last three years and maintain a valid Driver License during employment. 8. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) days of employment. 9. Must pass a drug and alcohol testing upon employment and random testing during the course of employment. 10. Must have a Tuberculosis (TB) skin test upon employment and employee health profile updated on an annual basis and must have updated Immunization (IZ) record NATIVE HEALTH is an EEOC, Employment-At-Will, and Native American Indian Preference employer.
