• Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
• Provide culturally appropriate services to members and their families
• Checks patient/clients in for appointments by reviewing and updating patient/client demographic information
• Performs insurance/eligibility checks on members by telephone or electronically as appropriate
• Obtains referral documentation (e.g consult notes, imaging/lab reports, procedures reports, etc.) for provider review and sign-off
• Actively outreach and link members to behavioral health, primary care, and community services, especially those who are historically hard to engage or not linked to health and social services.
• Facilitates and monitors scheduling and arranging of health and other wraparound service appointments for clients.
• Completes daily documentation into electronic health records to show coordination of care and updates on client health/wellness.
• Coordinates authorizations for referrals for NAC patients to outside specialists and/or services
• Support the patient/clients' individualized recovery action plans, crisis plans, and self-management plans.
• Provide one-on-one non-clinical interventions, serving as a member's advocate for needed health services and community resources.
• Adhere to regulatory requisites for quality assurance and documentation of interventions to warrant adequacy, completeness, and compliance with NAC's standards and all applicable legal, and ethical requirements.
• Reports monthly to a multi-disciplinary team the accomplishments and client outcomes from the previous month. Prepare quarterly reports from databases.
• Supports and actively engages in practices of inclusion through Cultural and Linguistically Appropriate Services (CLAS) Standards.
• Drives mobile unit to service locations.
• Other duties as assigned Qualifications: EDUCATIONAL REQUIREMENTS
• High School Diploma AND a minimum of five (5) years of proven work experience as a Community Health Worker or equivalent healthcare-related work experience, or;
• Associate degree AND a minimum of two (2) years of proven work experience as a Community Health Worker or equivalent healthcare-related work experience, or;
• Bachelor's degree – in social or healthcare-related degree with a strong understanding of the Community Health Worker model, or equivalent healthcare work experience required.
• A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges. WORK EXPERIENCE/SKILLS REQUIREMENT:
• Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings.
• Excellent verbal, written, and interpersonal communication skills; highly collaborative team approach to work and ability to work independently.
• Familiarity with formal and informal community-based organizations and resource networks
• Excellent customer service skills.
• Experience in working with the Native American population preferred.
• Must demonstrate critical thinking, problem-solving, and organizational and time management skills.
• Must be able to work well with others in a team approach.
• Possess and maintain a valid Arizona driver's license.
• Fingerprint Certificates within 90 days of hire.
• MS Office proficiency.
• Bilingual-Spanish desired.
• Know the economic, educational, and social problems of Native Americans and the referral sources available.
• Requires a proficiency level typically achieved with 5 years of Intensive Case management experience.
