Type: Tribal Salary Range: $43,645 to $65,467 / Per Year Open Period: 5/6/2015 to 5/6/2016 Summary: Serves as the key person in the care of patients diagnosed with diabetes by providing individualized diabetes self-management education and training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional Performance. Provides case management for clients with diabetes in an integrated ambulatory care center. Assures high quality care as demonstrated by clinical outcomes, patient, provider, and employee satisfaction. Qualifications: Licensed professional nurse with Certified Diabetes Educator (CDE) credentials; or who is eligible to become a certified diabetes educator. CDE preferred. And, 2 years of experience providing diabetes education. Bachelor of Science in Nursing preferred. Working knowledge of diabetes required. Towa speaker preferred. Basic Life Support (BLS) certification; CPR/First Aid certification or must obtain within 30 calendar days of hire. Must pass a medical exam. Must possess a valid driver’s license and be insurable through the tribe’s insurance carrier. Position is subject to a favorable criminal background check and motor vehicle record.
