• Facilitate scheduling of Diagnostic Assessments with providers
• Coordinate and maintain mental health wellness information
• Attend meetings affecting Native American clients both internally and externally regarding wellness
• Develop BHH Health Action Plans based on assessment results
• Develop and conduct BHH wellness groups based on traditional cultural principles and practices for all ages that support their wellness goals Qualifications: Qualifications: Meet DHS qualifications/definition of “Mental Health Practitioner:”
• 6,000 hours of supervised experience in the delivery of services to children and adults with mental illness, OR
• Bachelor’s degree in one of the behavioral sciences AND 2,000 hours of supervised experience in the delivery of services to children and adults with mental illness 5 or more years of experience providing case management, care coordination, and or mental health services to high risk clients Skill, knowledge, and experience with local American Indian cultural activities Work Type: Permanent, Full Announcement #: Client Care Navigator, MPLS Urban Who May Apply?
