The primary responsibility of a Case Manager at IHC is to assist clients in accessing community and healthcare resources and accurately assessing their needs. The Case Manager collaborates with clinic staff, community agencies, and clients to ensure they have access to medical care and local resources while ensuring continuity of care. This role is vital in helping IHC achieve its mission and goal of providing quality healthcare services to the community.
- Monitors client use of services and resolves potential barriers to access
- Provides orientation and in-service training to community agencies
- Maintains accurate participant records to aid service planning and delivery
- Prepares and submits reports as needed
- Conduct home visits, assess social service needs, and determine program eligibility with follow-up as needed
- Effectively plans and prioritizes a variety of projects and meets multiple deadlines
- Education: High school diploma or equivalent required (Bachelor’s degree preferred)
- Certification: May require to complete Indiana Navigator certification based on clinic needs
- Experience: Three years of experience in case management in an FQHC or healthcare setting
- Driver's License: Must possess an Active and valid Indiana Driver's License, including State-mandated Auto Insurance
- Travel: The ability to travel throughout the community is required
- Knowledgable of available community resources
- Excellent interpersonal and customer service skills
- Bilingual preferred, not required
- Computer-proficient
