Job Summary:
We are seeking a highly organized and dedicated Program Coordinator to oversee and support the management and development of individual support plans for our clients. This role is essential for ensuring the effective implementation of programs, services, and compliance with regulatory requirements. As a Program Coordinator, you will collaborate with clients' families, clinical teams, and direct care staff to monitor and review progress and ensure quality care.
Key Responsibilities:
- Support Plan Implementation: Ensure individual support plans are executed appropriately, with monthly service reviews to track goal progress.
- Service Oversight: Assist in the supervision and oversight of residential services and Family Support Workers (FSW).
- Collaboration: Work closely with clients’ family members, case managers, physicians, and the clinical management team.
- Program and Service Reviews: Analyze and summarize the status of progress, completing monthly and quarterly service reviews. Recommend program changes when necessary.
- Program Documentation: Review and ensure staff progress notes are accurate to support service billing.
- Budget Management: Monitor program activity costs, ensuring expenditures are aligned with the budget.
- Residential Activity Oversight: Ensure residential activities operate smoothly, including home appearance, cleanliness, staffing, and documentation.
- Employee Relations: Supervise and manage staff, including Home Managers and Direct Support Professionals. Oversee hiring, promotions, discipline, and terminations according to company policies and state regulations.
- Training and Development: Ensure that direct care staff are adequately trained, including ongoing staff in-service and client-specific training.
- Quality Management: Supervise and monitor the implementation of the Person-Centered Plan (PCP) and ensure compliance with state and federal inspections.
- Incident Reporting: Conduct investigations related to allegations of abuse, neglect, exploitation, or other safety concerns.
- Compliance: Ensure maintenance of the comprehensive record and address any deficiencies noted by regulatory agencies.
- Communication: Maintain regular communication with staff, the Independent Support Coordinator, and other members of the support team to ensure proper coordination of services.
- Safety: Conduct monthly safety checks and drills to ensure a safe environment for clients and staff.
- Census and Scheduling: Perform regular census checks and review timekeeping/schedules for direct support staff.
- Direct Support Coverage: Provide coverage for direct support staff shortages while maintaining program management responsibilities.
- On-Call Rotation: Participate in an on-call rotation, including weekends and holidays, as required.
- Additional Responsibilities: Assume additional duties as assigned, including assisting with community and state meetings, ensuring clients have adequate supplies, and more.
Qualifications:
- High school diploma or equivalent; relevant certifications or degree preferred.
- Proven experience in program management or related roles.
- Strong leadership and supervisory skills with the ability to manage a team.
- Ability to assess and resolve issues related to client care and program implementation.
- Knowledge of state and federal regulations related to program management.
- Excellent communication and organizational skills.
- Ability to work both independently and as part of a team.

Hours: Monday to Friday, 8:30 AM - 5:00 PM
