Overview: We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team as a Wound Care Specialist. In this role, you will be responsible for providing high-quality wound care and management to residents in nursing home settings. You will work alongside a multidisciplinary team to ensure the health and well-being of patients by assessing, diagnosing, and treating a variety of wound care needs. This position requires a strong understanding of wound healing, as well as the ability to communicate effectively with residents, families, and the interdisciplinary teams.
Key Responsibilities:
- Conduct rounds at nursing home facilities to assess patients with wounds, including pressure ulcers, diabetic foot ulcers, venous ulcers, and other chronic wounds.
- Develop and implement individualized wound care plans, including dressing changes, debridement, and other interventions in conjunction with the nursing facility’s nursing team.
- Collaborate with nursing staff and other healthcare providers to ensure comprehensive care for wound patients.
- Monitor patient progress, track wound healing, and adjust treatment plans as needed.
- Educate patients and families on wound care techniques, preventive measures, and lifestyle modifications.
- Document patient evaluations, treatment plans, and progress in electronic health records (EHR).
- Order and interpret diagnostic tests, such as imaging and lab results, to assess wound condition.
- Provide recommendations for additional treatments or referrals to specialists, as necessary.
- Ensure compliance with all infection control and safety protocols.
- Participate in ongoing education to stay up-to-date with the latest wound care advancements.
Qualifications:
- Active Nurse Practitioner (NP) Indiana license in good standing.
- Certification in wound care (e.g., Wound, Ostomy, and Continence Nursing Certification (WOCN) or Wound Care Certification) is preferred but not required.
- Strong experience in wound care, dermatology, or other relevant clinical fields.
- Excellent interpersonal and communication skills for interacting with patients, families, and the interdisciplinary teams.
- Ability to work independently and manage time effectively.
- Compassionate, resident-centered approach to care.
- Knowledge of geriatric care and the specific challenges of wound healing in elderly populations.
- Experience with electronic health records (EHR) and medical documentation.
