House Calls Nurse Practitioner
Abode Care Partners is seeking a compassionate and skilled Nurse Practitioner (NP) to provide personalized medical care to seniors and patients with complex needs in the comfort of their homes or care facilities.
About the Role:
- Provide evidence-based medical care to patients with chronic and urgent episodic illnesses, minor injuries, and other health conditions.
- Examine and treat patients, ordering laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals to develop and maintain comprehensive patient plans.
- Refer patients to other healthcare practitioners when necessary.
Requirements:
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience, including at least two years as a nurse practitioner.
- Active NP license and proficiency in medical instruments and equipment.
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
