House Calls Nurse Practitioner

Abode Care Partners is seeking a compassionate and skilled Nurse Practitioner (NP) to provide personalized medical care to seniors and patients with complex needs in the comfort of their homes or care facilities.

About the Role:

• Provide evidence-based medical care to patients with chronic and urgent episodic illnesses, minor injuries, and other health conditions.
• Examine and treat patients, ordering laboratory and diagnostic testing as needed.
• Collaborate with caregivers and other healthcare professionals to develop and maintain comprehensive patient plans.
• Refer patients to other healthcare practitioners when necessary.

Requirements:

• Master's degree in Nursing (MSN)
• Five or more years of nursing experience, including at least two years as a nurse practitioner.
• Active NP license and proficiency in medical instruments and equipment.
• Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
