Position Title: Case Management Consultant/Service
Location: Domus Infinita Pueris and Sun Park Haven
Position Summary: The Case Management Consultant/Service provides and oversees case management services for ICC programs, including Domus Infinita Pueris and Sun Park Haven. This role involves coordinating and administering services for individual children and youth, ensuring that their needs are met through effective case management. The position requires a commitment to maintaining high standards of practice, including ongoing professional development and compliance with accreditation requirements.
Role and Responsibilities:
- Case Management:
- Provide comprehensive case management services for children and youth within the ICC programs.
- Coordinate and oversee the delivery of services, ensuring that each individual’s needs are effectively addressed and managed.
- Coordination and Administration:
- Work closely with families, service providers, and other stakeholders to develop and implement individualized care plans.
- Administer and monitor the execution of these plans, making adjustments as necessary to achieve optimal outcomes.
- Documentation and Reporting:
- Maintain accurate and up-to-date records of case management activities and client progress.
- Prepare and submit reports as required, ensuring compliance with program and regulatory standards.
- Collaboration:
- Collaborate with internal teams and external partners to enhance service delivery and support.
- Facilitate communication between all parties involved in the care of children and youth.
- Training and Maintenance:
- Continuously update and maintain necessary training, experience, and skills to ensure effective case management practices.
- Stay informed about industry best practices and emerging trends in case management.
- Professional Accreditation:
- Ensure compliance with all professional accreditation requirements relevant to the case management role.
- Maintain any necessary certifications or licenses as required by the role.
Qualifications:
- Education and Experience:
- Master’s degree in a relevant field; or
- Bachelor’s degree with consultation from a master’s degree holder; or
- Four years of relevant full-time work experience in case management or a related field.
- Training and Maintenance:
- Demonstrated ability to maintain up-to-date training and professional development related to case management.
- Professional Accreditation:
- Proven track record of compliance with professional accreditation requirements relevant to case management.
Job Type: Full-time
Pay: $30.00 - $35.00 per hour
Expected hours: 40 – 45 per week
