Job Title: Care Coordination Nurse (Licensed Vocational Nurse)

Job Summary:
The Care Coordination Nurse (LVN) plays a vital role in ensuring seamless patient care by coordinating services, communicating with healthcare providers, and supporting patients in achieving optimal health outcomes. This position is responsible for assisting in the oversight of care plans, ensuring compliance with home health regulations, and serving as a liaison between patients, families, caregivers, and medical professionals. Additionally, the Care Coordination Nurse will manage Durable Medical Equipment (DME) needs to ensure patients receive appropriate supplies and equipment.
Key Responsibilities:
Assist in coordinating and overseeing patient care plans to ensure comprehensive and individualized treatment.
Communicate with physicians, therapists, home health aides, and other healthcare professionals to facilitate patient-centered care.
Monitor patient progress and report any changes to the Registered Nurse (RN) and healthcare team.
Educate patients and families on disease management, medication adherence, and home safety.
Assist in scheduling and coordinating home health visits to ensure continuity of care.
Review and process physician orders under RN supervision, ensuring accuracy and timely implementation.
Conduct follow-up calls to assess patient status and address any concerns.
Document all patient interactions and care coordination activities in accordance with agency policies and procedures.
Participate in quality assurance and performance improvement (QAPI) initiatives to enhance patient care services.
Serve as a resource for field staff, providing guidance on clinical concerns and regulatory compliance.
Maintain knowledge of Medicare, Medicaid, and private insurance guidelines relevant to home health care.
Ensure timely submission of documentation and compliance with state and federal regulations.
Coordinate Durable Medical Equipment (DME) needs, including ordering, tracking, and ensuring timely delivery of necessary medical supplies.
Educate patients and caregivers on the proper use and maintenance of DME.
Work with vendors and insurance providers to facilitate DME approvals and cost coverage.
Follow up on all patients who are transferred to inpatient facilities to ensure continuity of care and a smooth transition back to home health services if needed.
When intake is out, LVN will be assisting.
Qualifications:
Licensed Vocational Nurse (LVN) license in the state of practice (required).
Minimum of one year of nursing experience, preferably in home health, case management, or care coordination.
Knowledge of home health regulations, OASIS documentation, and Medicare guidelines preferred.
Experience with Durable Medical Equipment (DME) management preferred.
Strong communication and organizational skills.
Ability to work independently and collaboratively in a team environment.
Proficiency in electronic medical records (EMR) and Microsoft Office Suite.
CPR certification (required).
Physical Requirements:
Ability to travel to patient homes and office locations as needed.
Ability to sit, stand, and walk for extended periods.
Ability to lift and carry medical equipment and supplies up to 25 lbs.
Schedule:
Full-time or part-time, with flexible scheduling based on patient needs.
