Position: Care Coordination Coach
Reports To: Program Manager
Department: Disaster Recovery Services
The Care Coordination Coach will support Care Coordinators in delivering high-quality service to residents of Los Angeles County by conducting needs assessments, providing appropriate referrals to designated organizations and navigating clients to resources, programs and activities. The Coach will continuously guide and instruct staff to ensure that individual and agency performance standards are achieved.
- Provide leadership, guidance, communication, coaching, instruction, and professional development to Care Coordinators.
- Offer guidance, orientation and training to Care Coordinators to perform their job.
- Partner with the Program Manager and other leadership staff to develop improvement plans for Care Coordinators.
- Promote the organization's values and strategic plans while cultivating an environment of trust, teamwork, and professional growth.
- Collaborate with program managers and directors to ensure proper staff scheduling and adherence to protocols.
- Develop and implement processes and systems that support efficient and effective service delivery while ensuring compliance with operational policies and regulations.
- Assist Care Coordinators in resolving complex client inquiries and provide guidance on best practices.
- Conduct regular coaching sessions with each team member to target opportunities for professional growth and build morale.
- Develop and implement programs to motivate employees and foster a strong sense of team collaboration and accountability.
- Conduct regular performance evaluations for all direct reports.
- Facilitate meetings to communicate team performance goals, share updates, and address challenges or opportunities for improvement.
- Analyze department performance and proactively identify and implement strategies to enhance quality, productivity, and morale.
- Respond to team members' needs with a positive and supportive attitude, building constructive and effective rapport.
- Actively use program data to participate in strategic planning and process improvement discussions, contributing insights and recommendations.
- Perform special assignments and other duties as assigned in alignment with the agencys mission and values.
- Four-year college degree or equivalent work experience in a leadership or management role within a care coordination or case management.
- Basic knowledge of labor laws and workplace best practices.
- Experience with communication systems, electronic healthcare records, resource databases.
- Understanding of agency mission, procedures, and best practices relevant to the role.
- Strong organizational skills with the ability to manage multiple priorities effectively.
- Excellent oral, listening, and written communication skills.
- Ability to work effectively with people from diverse backgrounds.
- Proficiency in computer software and systems relevant to the role.
- Valid drivers license and proof of insurance if using a vehicle for official agency business.
- Ability to travel as needed and work evenings or weekends as required.
- Availability for short-notice scheduling in response to emergency situations or disasters.
- Working knowledge of human services and service delivery models.
- Demonstrated supervisory skills sufficient to manage up to 15 employees.
- Ability to support and encourage staff, recognize contributions, and foster a positive team environment.
- Strong initiative, problem-solving, and project management skills.
Compensation: $68,640 annually
