Ingleside Communities is seeking to hire an Admissions and Care Experience Navigator Nurse to join our amazing, dedicated team!
The Admissions and Care Experience Navigator Nurse will focus on providing new patients/residents and families with the necessary coordination of services for an optimal and seamless transition into the facility. This position manages the facility’s internal admissions process from intake to discharge and is responsible for coordination of all tours and admissions, to ensure the process is thorough and the patient has a smooth admission, with a focus on their individualized clinical needs and comfort.
This is a full-time, on-site position at our community located at 407 Noth 8th Street, Mt Horeb, Wisconsin.

Position Responsibilities:
- Works closely with Admissions and Care Transitions Manager to drive admissions and ensure a smooth admissions process for patients.
- Partners with Nursing, MDS, Social Services and related departments for coordination of admissions, bed availability, census changes or room changes.
- Completes PAN Census for PAN participating facilities.
- Responsible for completing PASRR.
- Completes census events in MatrixCare for admissions, discharges, and transfers.
- Ensures that the patient room has been prepared and does final check to assure that it is neatly prepared for receiving the admission.
- Arranges for equipment with Central Supply (DME equipment, specialty beds, etc.) and ensures that it is present in the patient/resident room prior to admission.
- Acts as the patient/resident/family liaison at time of admission and throughout their stay.
- Initiates greeting and/or coordinates the process for new admission and explains what will occur during the admission process.
- Completes admission agreement and consents with resident/POA.
- Responsible for initial admission assessments and medication reconciliation.
- Follow up with patients at regularly scheduled intervals (to be defined upon) to ensure facility is meeting and succeeding expectations.
- Responsible for insurance revalidations, as needed.
- Provides NOMNC to patients, as needed.
- Work with social services to ensure a safe, stable and successful discharge to the community or next site of care.
- Individual responsible for community outreach and working with ALs.
- Primary person for facility tours and ensures that the facility is consistently tour ready.
- Ensures follow up on all tours, leads, and open inquiries until closed, in coordination with Admissions and Care Transitions Manager.
- This position to include on-call rotation weekend coverage.
Requirements:
- LPN or RN licensed in the State of Wisconsin.
- Must have a maintained current CPR certification.
- Minimum 2 years of experience in healthcare, post-acute or long-term care.
- Ability to relate positively, effectively, and appropriately with residents, patients, families, community members, volunteers and other staff members.
- Able to read, write and speak English.
- Basic computer knowledge and ability to learn company applications.
