Wisconsin Illinois Senior Housing, Inc. is actively seeking to hire an Admissions and Care Transitions Manager Nurse to join our team. This position will oversee our Ingleside Communities, Edgerton Care Center and Montello Care Center facilities in this role. The role will be based at our Ingleside Communities facility, located at 407 North 8th Street, Mount Horeb, Wisconsin.
The Admissions and Care Transitions Manager’s primary role is to triage and review new referrals and secure new patients and residents through collaboration with the key referral partners. Responsible for continuous communication about prospective referrals, facility admission decisions, and coordination between the referral site and the facility admissions director to ensure a smooth transition for the patient/resident and family.
This is a full-time, hourly, on-site position.

Position Responsibilities:
- Lead team member for assessment of admissions.
- Oversees all inquiries and referrals professionally, timely and with appropriate follow-up.
- Responsible for daily professional communication with both external partners (hospitals and other referral partners) as well as internal facility staff.
- Hospital and referral partner liaisons as to bed availability throughout the day.
- Responsible for ensuring all post-acute and referrals are converted to admissions as soon as possible including following up to ensure that all scheduled admissions for the day are closed.
- Responsible for initial insurance verification
- Maintain database of existing and potential referral sources. Utilizes sales practices and sales tracking tools.
- Oversees the obtaining of all background checks on new patients.
- Works closely with the Admissions and Care Experience Navigator to ensure a smooth transition to the facility.
- Notifies the facility team of all new admissions. Alerts appropriate department heads and building staff of projected changes, i.e. admissions bed changes and upcoming transfers.
- Maintains a close working relationship with the Administrator, DON and the Business Office Manager to assure appropriate and efficient decisions to facility management.
- Communicates with nursing and housekeeping about appropriate room and bed selection when admissions determinations are made
- All other duties as assigned
Requirements:
- LPN or RN licensed in the State of Wisconsin.
- Must have a maintained current CPR certification.
- Minimum 2 years of experience in healthcare, post-acute or long-term care.
- Ability to relate positively, effectively, and appropriately with residents, patients, families, community members, volunteers and other staff members.
- Able to read, write and speak English.
- Basic computer knowledge and ability to learn company applications.
