Licensed Master Social Worker
As a Licensed Master Social Worker, you will play a vital role in supporting patients with End Stage Renal Disease (ESRD). Your primary responsibility will be to help patients understand their rights and responsibilities, as well as guide them in managing the physical, mental, emotional, and financial demands of ESRD.
Key Responsibilities:
- Assess patients' needs and develop personalized care plans
- Educate patients on self-care techniques and disease management
- Collaborate with healthcare teams to ensure comprehensive care
Requirements:
- Master's degree in Social Work from an accredited institution
- Current state licensure (e.g. LCSW, LMSW) that meets state regulations for practice in a dialysis setting
- At least two years of clinical work experience, preferably in an ESRD setting
- CPR certification preferred
- Excellent communication and interpersonal skills
