About Us:
The Mission of Yahweh is a faith-based nonprofit dedicated to empowering homeless women and children by providing shelter, support services, and programs that promote self-sufficiency. We are seeking a compassionate and motivated Case Worker to join our team and make a meaningful impact in the lives of our residents.
Position Overview:
The Case Worker will provide direct support to residents by conducting assessments, developing individualized service plans, and connecting them to community resources. This role requires strong communication, problem-solving, and advocacy skills to help residents achieve stability and independence.
Key Responsibilities:
- Conduct intake assessments and develop personalized service plans for residents
- Provide case management services, including goal setting, progress monitoring, and referrals
- Assist residents in accessing resources such as housing, employment, education, and healthcare
- Collaborate with internal departments and external agencies to coordinate services
- Maintain accurate and confidential case notes and documentation
- Advocate for residents’ needs while fostering self-sufficiency
- Facilitate life skills workshops and support groups as needed
- Participate in team meetings and contribute to program development
Qualifications:
- Bachelor’s degree in Social Work, Human Services, or a related field required
- Minimum of 2 years of case management or social services experience
- Strong understanding of trauma-informed care and community resources
- Excellent interpersonal, organizational, and documentation skills
- Ability to work independently while being a collaborative team player
- Proficiency in Microsoft Office and case management software HMIS
